CASE MANAGEMENT
For those who sustain a catastrophic injury including spinal cord injury, amputation, brain injury or serious multi trauma orthopaedic injuries, life is never the same again. Our case managers help individuals to cope with their disabilities and rebuild their future.

IMMEDIATE NEEDS
A complete and comprehensive Initial Needs Assessment (INA) is carried out by a case manager, to identify an individual’s needs in every aspect of their life. A detailed Initial Needs Report (INR) making recommendations and determining costings is then provided. Regular update reports are then produced which detail ongoing interventions and progress in all areas of need.

REHABILITATION
All physical, psychological, social, and environmental needs are addressed by our case managers to maximise an individuals independence. Case Managers implement suitable interventions, oversee equipment, housing and transport provision and monitor theraputic involvement. Utilising specialist contact networks, and their clinical skills our case managers provide expert support to individuals.

CARE PROVISION
Our case managers are experienced in the organisation of care packages from initial provision to long term stability. This includes the recruitment and supervision of support workers. They will assess each individual’s needs and, using a problem solving approach, ensure a care solution is tailor made for them.
